Forms You Need:

If you've paid your registration fee, you will need to complete and submit the following forms.  Because the forms require signatures, please mail them to us or bring to us on site.  If you are familiar with digital signatures, you may also digitally sign them, using a computer owned by you.

1. Child Information Record & Contract - you may need to refer to the Rate Card so you know the proper charges (or call us if you are unsure).
