To Register, please complete the registration form, save it to a file, and email it to us. You can mail your registration check or give us a call to make a credit card payment at 734-697-1566 (do not email any credit card information).
You ARE NOT registered until we receive your registration payment or deposit.
You will need Adobe Reader to view and print any of our forms and publications.
