Before working at M&M, it is a requirement of Childcare Licensing for all staff to be fingerprinted for a comprehensive background check. A specific form, CCBC - Michigan Child Care Background Check Consent and Disclosure, must be completed by the applicant. Once completed, an administrator of M&M will contact you to set up your fingerprinting appointment. No worries - we cover the cost of fingerprinting, you just need to be there (with valid identification, of course). Click below to complete the fingerprint authorization forms:
Required Forms is a packet of forms and necessary tasks that MUST be completed before your first paycheck can be processed. If you cannot complete these before the next pay period deadline, you will have to wait until the next pay period (two additional weeks) for your first check. Don't forget to submit the W-4 also.
A negative TB test within the last 12
months, or within 2 weeks of hire must be provided (however, your hours will not be submitted to
payroll until the test has been completed).
Please reach out to us if you need a place to get tested. Wayne County has a TB Clinic and costs just $10. Your other options could be a Minute Clinic or nearby urgent care. Do NOT go anywhere that costs more than $25, as cheaper options are available.

Licensing Requires that ALL individuals caring for children must complete Health and Safety Training via the MiRegistry Website. This is a four-part training series and your Site Leader will help you register. Currently, there is no cost to register. All required training and continuing education will be accessed and managed through MiRegistry and Michigan Virtual. In addition, be prepared to register for an upcoming Adult/Child CPR-First Aid Course within the first 60 days (if you are not already certified).
